Please use the form below to send inquiries. Our office will respond to your request in writing via USPS mail.
To ensure the security of all CPF plan participants, we will NOT be able to process any request for changes to CPF records (i.e. address changes, bank changes, estimate requests, etc.) via the Contact Us page. If you would like to make such a change, please download the appropriate form from the website and either mail or fax it to our office.
Please note, all such forms must be completed in their entirety before submitting them to the Fund's office. CPF is unable to process forms with missing information.
